He told her that he was paying her mother's medical bills and suggested that she turn off her mother's life support. Claire was so upset she left her father without learning his name.

Claire then changes her mind and walks out before signing the adoption papers. She visits Malkin one more time and he tells her that there is a couple in Los Angeles who are prepared to adopt her baby. He gives her a ticket to Oceanic Flight 815.
After crashing on the island Claire becomes friends with Charlie. She also realizes that Malkin foresaw the plane crash and wanted her to go the island, forcing her to be the one to raise her child. Alex saves her and leads her into the jungle. She is later rescued by Rousseau and returned to the survivors' beach. Claire has no memory of what happened to her while she was kidnapped, but her memories slowly return.
After Claire left, Ethan told Jack and the others that he would kill them if Claire wasn't returned. The other survivors avoid Claire, but Shannon tells her about Ethan's threats. Claire agrees to be bait for Ethan, who is then killed in an ambush by Charlie, in retaliation for what he did to Claire.

Rousseau later kidnaps the baby, hoping to trade it for her own baby who was taken by the Others, but Sayid and Charlie save the baby. Claire chooses to name the baby Aaron.

Aaron later falls ill and Claire asks Libby to hypnotize her to reveal her repressed memories. She, Kate and Rousseau later go to find the Staff station and the vaccine. Unfortunately, the station is empty. Before returning to the beach, Claire tells Rousseau that her daughter, Alex, is still alive and helped save her. Aaron recovers from his illness and Claire has recovered all of her memories.

After contacting the freighter, Claire and the other survivors head back to the beach. Along the way she learns of Charlie's death and decides to join Locke's group in Othertown, along with Hurley and Sawyer.
